Positioning- The Secret Sauce of Success. by The Evidence Based Chiropractor

Marketing guru's such as John Carlton teach 3 kinds of positioning.  Positioning your practice appropriately within your local community is perhaps the best long term path to success.  There are great, actionable, lessons that can be learned from each type of positioning. 

1-  The first type of positioning is establishing your "eye level".  In retail, this means literally having your product at eye level.  It is the best and most direct indicator of sales.  Relating this to interdisciplinary relationships; we should have our practice at "eye level" of other physicians in the community.  We should be engaging them on a consistent basis through Case Notes, Monthly Research Briefs, and occasional meetings.  This keeps your practice at "eye level" and not hidden on the back of the shelf!

2-  Second is where your "ad" appears in magazines and publications.  Ad agencies long ago learned to charge a premium for the first dozen pages of advertising in their publications.  Of course this is because while 90% of readers will look through the first dozen pages; not even 10% will make it to the end of the publication.  Our lesson to be learned from this is to keep our communication short and poignant.  Physicians are not going to read 10 page research papers, or 15 page case notes forms.  Quite simply, it is important to provide one page updates that grab their attention.  

3.  How you position your service in the "readers" mind.  This is perhaps the most obvious with how it relates to your practice.  Does your practice exude professionalism?  Do people in the community know that your office works with (not against) other physicians in town?  Have you positioned your office as the only logical choice for primary care physicians to make chiropractic referrals?  

As you can see, how we position our office will make a huge difference in our success not only with interdisciplinary referrals, but also with our friends and neighbors.

Epidural Steroid Injections Performed No Better than Lidocaine Injections- by The Evidence Based Chiropractor

This large, multi center study published by the New England Journal of Medicine found that, for patients with lumbar stenosis, injections of lidocaine performed just as well as injections containing lidocaine and corticosteroids.  The study was double-blinded and randomized.  

“Compared to injections with local anesthetic alone, injections with glucocorticoids provided these patients with minimal or no additional benefit.” said the author of the study, Janna L. Friedly, MD.

This study may have important implications.  Earlier this year we touched on a study which showed that ESI's have greatly increased the risk of lumbar vertebral fracture.  Additionally, ESI's have a fairly high complication rate and their effectiveness is questionable at best when compared to other conservative care options.

The UK BEAM Study: A Landmark Paper for Spinal Manipulative Therapy- by The Evidence Based Chiropractor

In 2004, the British Medical Journal published the UK BEAM (United Kingdom Back Pain, Exercise, and Manipulation) study.  This was a randomized trial to test the effectiveness of various physical treatments for back pain.  The study showed that manipulation provided the greatest response to care for patients with un-resolving lower back pain; and if cost or time is an issue, manipulation alone provided a greater net benefit than exercise alone.

Manipulation/Adjustments alone had statistically significant improvement in the Roland Disability Score for the manipulation group at 3 months (1.57) and remained significant at 12 months (1.01).  The exercise only group was significant at 3 months (1.36) and not at all at 12 months (.39).  When examining the treatment groups, the most robust separation from the baseline Roland Disability Scores were the treatment groups that included manipulation.  Additionally, you will notice that no serious advance effects were recorded.

Cardiovascular Risk Increased for Children Taking ADHD Medication- by The Evidence Based Chiropractor

New Danish research shows that children taking medication for ADHD are at a greater risk for cardiovascular problems than a sample of children with ADHD who are not taking medication.  The risk for cardiovascular problems was found to be doubled for those taking medications. 

The absolute incidence of these potential problems is still very low (under 1%), however, it is alarming anytime the rate of potential pathology is doubled due to a medication.  With literally millions of children being prescribed ADHD medications each year; there is the potential for these medications to cause a significant number of children to encounter cardiovascular problems and be forced to receive more intensive treatments and medications.
